A minimum donation of $25 is required for personalized memorial plaques.  Donations received for personalized plaques will go to IMOM's General Fund to help in emergency PIN cases.

On this page you may choose the background you would like to have and place your order.  After you have placed your order you will be taken to a new page where you may make your donation and find instructions for sending your photo. 

We will do our best to get your plaque made within 48 hours after your donation has been received.  When your plaque is completed you will receive an email with a copy of your plaque attached and a link to the page where it is displayed at IMOM.
