At IMOM we have six different applications.  You must decide which application pertains to your particular situation.  The appropriate application selection will determine the following steps your specific needs.

Emergency	Emergency applications may be requested only after IMOM has received confirmation from the attending veterinarian that the animal will die or have to be euthanized within 24 - 48 hours if not treated.  You will be provided with step-by-step instructions.  Take a deep breath and try to relax.  We're here to help you and your companion get through this crisis. General Fundraising and Visually Impaired Pets	The General Fundraising Application covers a wide range of uses.  Its primary use is for applicants whose pet is in need of treatment but their condition is not presently life threatening and we have time for fundraising.  The General Fundraising application is also used for visually impaired pets who are in need of non-routine veterinary care. The entire pre-application procedure will be thoroughly outlined for you using a step-by-step method. If you qualify for financial aid and your application is approved, IMOM will give you an opportunity to fundraise through our online community.  IMOM will not normally make a financial pledge until you have shown our committee and our community that you are doing everything you can to help your pet. Upon approval of your application IMOM will begin to solicit donations for your pet.  In addition, you will have the opportunity to meet many of our supporters at our Online Community Forum.  It's quite an experience! When all steps are followed properly IMOM can usually have a fundraising page up within 24 hours after your application is received and approved.  Don't forget to have a photo available to send via email when requested. Pets in Rescue	This application is used exclusively by rescue organizations.  The pet must be in the care of the rescue at the time the application is submitted.  The rescue must be a 501(C)3 as one of the qualifications. Click here to continue if you are a qualified rescue.
