Step 5 - Documents IMOM needs before you request the application.

To determine which documents you will need to send to IMOM please see the list below and click on the option that best describes your situation.

  1. Documents IMOM needs if you are required by IRS to file taxes. Click here

If you ARE required by the IRS to file taxes the following documents are needed before you may complete an application:

  1. As 1st proof of income -- A complete copy of your latest tax return.  If you and your spouse/partner file separately, we need copies from both parties. All pages must be sent.

    FOR YOUR PROTECTION - Before sending any documents to IMOM you must black out all occurrences of account numbers, including Social Security number.  Documents including account numbers and/or Social Security numbers will be destroyed and you will have to re-send all documents.

  2. As 2nd proof of income -- Copies of your last two bank statements from all bank accounts for both you and your spouse/partner.  All pages must be sent and must reflect payment of monthly living expenses.

    If the bank statement you send does not reflect payment of monthly living expenses you are required to send copies of payment receipts. We will need receipts for 2 or more of the following:

    Rent or mortgage - Telephone - Insurance (home, life, health or auto) - utilities - Internet - cable or satelite TV - any other monthly expenses you pay.

    FOR YOUR PROTECTION - Before sending any documents to IMOM you must black out all occurrences of account numbers, including Social Security number.  Documents including account numbers and/or Social Security numbers will be destroyed and you will have to re-send all documents.

  3. You are required by IMOM to apply for Care Credit if it is accepted by your veterinarian. If you applied for Care Credit IMOM needs a copy of the letter of approval or denial.  Before sending any documents to IMOM you must black out all occurrences of account numbers, including Social Security number.   *Approval from Care Credit does not necessarily mean you will not qualify for financial aid from IMOM. You are invited to continue with our application process*

  4. A completed Veterinarian's Pet Information Form (VPIF) - If you are following step-by-step instructions you will already have this document completed by your veterinarian.  It is step 4 in the process.

  5.  A written estimate from the veterinarian who will be treating your pet for its current condition.

    The estimate must be on your veterinarian's letterhead and include the following information or IMOM cannot accept it:

    (a)  Your name and the name of your pet.
    (b)  A complete breakdown of costs for one-time treatment. 
    (c)  If your pet is not spayed or neutered that cost should also be included in the written estimate.

How to send your documents to IMOM - there are two methods to choose from

Before sending any documents to IMOM you must black out all occurrences of account numbers, including Social Security number. 

  1. You may send your documents via fax. Please be certain to have all pages turned so we do not get the blank side of the document. Our fax number is: 1-630-214-8952

    If you or your spouse do not have a bank account your are required to send copies of receipts for payment of monthly living expenses

    FOR YOUR PROTECTION - Before sending any documents to IMOM you must black out all occurrences of account numbers, including Social Security number.  Documents including account numbers and/or Social Security numbers will be destroyed and you will have to re-send all documents.I

  2. As 2nd proof of income -- one from list below for both you and your spouse/partner.

       A copy of your most recent pay stub.
       SSI benefits letter.
       Proof of any Federal or State assistance
       Unemployment benefits letter.
       W-2 or 1099 from employer.

  3. You are required by IMOM to apply for Care Credit if it is accepted by your veterinarian. If you applied for Care Credit IMOM needs a copy of the letter of approval or denial. 501(c)(3) rescue organizations

Documents needed:
  1. A written estimate from the veterinarian who will be treating your pet for its current condition.

    The estimate must be on your veterinarian's letterhead and include the following information or IMOM cannot accept it:

    (a)  Your name and the name of your pet.
    (b)  A complete breakdown of costs for one-time treatment. 
    (c)  If your pet is not spayed or neutered that cost should also be included in the written estimate.

  3. Copy of your adoption application.  If your adoption application is on your web
    site you may send us the link to it.

  4. Copy of your adoption contract.  If your adoption contract is on your web site you may send us the link to it.

  5. Reference from your veterinarian.

  6. Copy of your letter from IRS confirming your 501(c)(3) status.

  7. References from at least two people who have adopted from your organization.
