Step 7 - Confirm that IMOM has received your documents and request an application.
When all of the documents outlined in step 5 have been sent to IMOM, you may ask for confirmation that IMOM has received them and request an application.
Please understand - this form IS NOT the application. By submitting this form you are asking for confirmation that IMOM has received your documents so you can complete an application. When the Financial Aid Committee receives your request we will check our fax archive to see if we have the necessary documents. If we do, we will send you a link to our online application. If we do not have the necessary documents you will be advised accordingly.
Caution - We absolutely cannot send you the application until we have received the required documents listed in step 5. Do not complete this form before you have sent the required documents. You will only slow down the process and postpone any help we might be able to give your pet.
