When you don't read the information we have provided for you, you are placing limits on our ability to help your pet in a quick and efficient manner.

Due to the enormous volume of email we receive, please be advised that you may not get a reply if your question can be easily answered by reading the information already provided on our site and FAQ page.

If you get a confirmation page after submitting the contact form, you will know your question has gone to the Financial Aid Committee.

All fields in the form must be completed or it will not submit.

Outline of steps to financial aid

Before you begin, read the introduction page.
Step 1 - Join our community.
Step 2 - Apply for Care Credit.
Step 3 - Determine whether or not you qualify for financial aid from IMOM.
Step 4 - Documents you need to give to your veterinarian.
Step 5 - Documents IMOM needs from both you and your veterinarian.
Step 6 - Read our policies.
Step 7 - Ask for confirmation that IMOM has received your documents and request an application.
Step 8 - IMOM will confirm receipt of the required documents. If everything is acceptable you will be sent a link to our application to complete online.
