The form below is NOT the application.  It is a request for an application.

After faxing all the necessary documents to 630-214-8952 and completing this form you will need to watch your email very closely for correspondence from a volunteer for IMOM's Financial Aid Committee.  The email will contain instructions you'll need to complete the application for emergency care. 

If your veterinarian has not advised IMOM in writing that your pet will die or have to be euthanized within 5 days or less, you do not qualify.

After submitting this form you will get an email from a committee volunteer.  Promptness of the email depends on time zones and availability of a volunteer.

You can get help much more quickly if you complete this entire request form.  Incomplete request Forms cannot be accepted.

Necessary documents

As a registered charity imom is required to have certain documents on file. 

NOTE - We cannot send you an application until you have faxed all necessary documents to
630-214-8952 

Have the following documents been faxed to IMOM?  If not please do not continue. 

  1. A written estimate from your veterinarian for the full cost of treatment needed.  The veterinarian must also advise IMOM whether or not your pet is facing death or euthanasia if not treated within 5 days or less.

  2. Have you or your veterinarian faxed the Veterinarians Pet Information Form (VPIF) to IMOM?
