Financial Aid - Emergencies (only)


 

As much as IMOM wants to help, you must keep in mind that we may not have the funds necessary to help your pet.  If you will work very closely with our volunteers, we will always do all we can to help you and your pet.

 

Is your pet in critical condition and in danger of death or euthanasia in the next 48 hours due to their medical condition, not your lack of funds?

Take a deep breath and try to relax.  IMOM is here for one primary purpose.  That is to help people help pets.  IMOM can give hope where there had been no hope, because you simply do not have the money you need.  If you qualify for financial aid we will do all in our power to help your pet.

When your veterinarian has examined your pet and determined that treatment must take place within the next 24 to 48 hours, IMOM considers it to be a life threatening emergency.  These are the only conditions under which we can send you an emergency application form.

By following the instructions on this page the entire emergency application process can be completed in a short period of time.  Quite naturally, this depends on the time of day your application is submitted and availability of volunteers.  We are available every day but not 24 hours a day.

There are four steps outlined below.  Read them very carefully.  It could mean life or death for your pet.

Documents sent to IMOM must have all personal account information blacked out.  This includes social security number, bank account and bank routing numbers as well as any other personal account numbers.

If account numbers are visible IMOM will immediately destroy all of the documents you have sent.  If this happens you will be required to send all documents again.

Apply for Care Credit

Ask if your veterinarian accepts Care Credit.  Care Credit is not a part of IMOM.  If they do accept Care Credit you are required by IMOM to apply.  Even if you do not get approved for the full amount your pet needs.  Most veterinarians who accept Care Credit are set up so you can apply from their office.  You may also apply Online.

Everyone does not qualify for financial aid from IMOM.  Take a couple of minutes to see whether or not you qualify.

There are 4 documents you need. 

These documents must be faxed to IMOM
before we can send you an application form.

Document  #1 - Estimate of costs

Get a written estimate from the veterinarian who will be treating your pet for it's current condition.

The estimate must be on your veterinarian's letterhead and include the following information or IMOM cannot accept it:

  1. Your name and the name of your pet

  2. A breakdown of costs for treatment and after care. 

  3. If your pet is not spayed or neutered that cost should also be included in the written estimate.

  4. How soon treatment should be provided to save the animal's life.

  5. Your pet's prognosis.

  6. The veterinarian also needs to confirm in writing that they will work within IMOM's payment policy.  If your veterinarian is not familiar with IMOM, we have 3 documents you may print and take to them.  Click here to find and print the letters.

  7. Your vet must indicate whether or not Care Credit is accepted at their facility.

Document #2 - proof of household income

IMOM will need proof of income for all household members.  Before sending any personal documents to IMOM please be sure to black out any account numbers and/or your social security number.

Acceptable proof of income would be one of the following:

  • Always required - A copy of your most recent tax return if you are required by IRS to file.

  • A copy of your most recent paycheck stub.

  • SSI benefits

  • Unemployment benefits

  • W-2 or 1099 from employer

  • A copy of your most recent bank statement.  The statement can be from either a checking or savings account.

Document #3 - Letter of denial from Care Credit

  • Letter of denial from Care Credit if you are required by IMOM to apply for Care Credit.  You are required to apply for Care Credit if it is accepted by your veterinarian.

    *Approval from Care Credit does not necessarily mean you will not qualify for financial aid from IMOM.  You are invited to continue with our application process.*

Document #4 - a signed agreement from your veterinarian to acknowledge their willingness to work with IMOM payment policies.

Step four:

Request the Emergency application

After you have faxed the necessary documents to IMOM you may request an application for emergency veterinary care. 


This will not be the application.  This is a request for IMOM to send you the application after you have completed steps 1,2 and 3 shown above.
